SAFE’s Incident and Forensic investigations are designed to identify any available defence, mitigate criminality, prevent a reoccurrence and engage employees and contractors to improve compliance and injury prevention across the business.

SAFE undertakes investigations regularly throughout South East Asia and Europe and has conducted over six hundred Investigations, averaging over eighty matters per year. Investigations include near hit, permanent damage type injuries, fatality and multiple fatality matters and have seen

SAFE perform investigations across the world including New Zealand, Papua New Guinea, Thailand, China, Singapore, France, Finland, Sweden, Turkey, Germany and Hong Kong.

SAFE’s investigations are conducted and managed by Tertiary Qualified Safety/Forensic Engineers, former Regulator Inspectors and supported where required by qualified crime scene investigators.

SAFE’s approach is unique to the market and ensures professionally managed investigations and comprehensive reports are tailored to the specific objectives of the task with opinions supported by Professionally and Technically Competent personnel.

Previous results have included:

·        Identification of the correct root cause for the incident which resulted in the matter being discontinued by the Regulator.

·        Identification of evidence that had been over looked by the Regulator’s inspection/investigation team which proved component failure as opposed to system breaches.

·         Location of eye witnesses that observed the failure of critical plant seconds before a major conflagration.

·        The testing and FEA of critical equipment to determine failure routes.

·         Prosecutions being successfully defended.

·        Charges being withdrawn by the Regulator before the commencement of the matter in court.

·         Mitigation of fines.

·        Directors and senior managers being protected against personal prosecution.
